Leland van den Daele, PhD accepts all PPO insurance plans with out-of-network benefits. For out-of-network clients, he provides a monthly “superbill” for insurance remuneration.

- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Co-pay or co-insurance amounts
- Pre-authorization requirements
